What will you do in the study?
- Meet with the research team for an educational session about the study and diabetes care
- Meet with your health care provider as usual, complete surveys to document progress
- Track your medication use, blood pressure, and blood sugar
- Read and respond to text messages sent to your cell phone
Who is eligible to participate?
- Adults aged 18 or older
- Receive primary care at one of our 12 participating clinics
- Diagnosed with diabetes
- HbA1c ≥ 8.0%
- Taking a blood pressure or cholesterol medication
- Speak English fluently
- Be able to provide informed consent
What will participants receive?
- $20 dollars for the first visit and for each of five follow up visits. There is also a bonus for completing the study
- Blood Pressure Monitoring Kit for duration of study
- Educational material and cell phone reminders about blood pressure, blood sugar, medication, healthy lifestyle, and risk of heart disease
- Appointment reminders
